I'd say you have found something for the "you aren't going go believe what happened..." catagory.
Just curious, was any work performed on that hose before the first time it came off?
Any who, I'd consider doing the following"
1) drain the radiator low enough so you can work on the hose and radiator and keep them clean.
2) check the hose clamp, make sure it's in great condition or replace it. (no crust, free movement of the clamp screw down)
3) clean rag, spray some brake cleaner on the rag, grab the radiator connection with the rag and twist it like takein a top of a jar. spray again and do the *inside* of the hose. antifreeze is some slippery stuff so that should have cleaned off from previous blow outs. put the hose on the radiator and *snug* that clamp. NOTE: make sure the hose is pushed all the way on the pipe, a saab hose has a notch in it that will align with a tab on the hose. Make sure when you snug the clamp that has been pushed back on the hose far enough so that it's completely over the radiator inlet pipe. If you have it part way, or horrors not over at all, it could blow off.
refill the coolant, start, do a tour around the block, park and let cool, recheck coolant level.
Let us know how it goes and report back every once in awhile to let us know all is well!
