Sounds like you hit the nail on the head. Happened to me as well, but I didn't know the history of my slave. I can only assume a small particle was dislodged sometime in the last 3200 miles and scored the cylinder wall of the slave. The bad thing is you can't tell until you have it out and by then you might as well put a new one in. I always flush the lines really well after a replacement and have not had one go since. Could be a coincidence but it only costs a bit of fluid, so I'll keep doin' it.
There is also a problem I've read about with older 9000's where the rubber hose that feeds the master cylinder breaks down and contaminates the lines. I think that leads to master failure though. It's a $30 part... I replaced it on my last failure just because I'd done everything else including the Aeroquip stainless slave line trick.
