Its best to replace them if they have been in there for a long time. They can be partially cracked and fail with the last twist on the wrench or after running a while longer.
The studs can be drilled out in the car with a 90 degree drive and a centering sleeve in the manifold stud hole(s).
When replacing the head, its best to have the exhaust manifold off the head, and the studs out, then installing the studs after the manifold is on the turbo and the head in place. Even then the manifold may not fit over the turbo studs with the head in place, without filing the manifold holes a bit oblong in the appropriate places, or placing the manifold on the turbo before mounting the head.
