The second flapper is directly above the first, and is connected with a linkage that you can't see with the fan box installed.
I've never seen an end-tank failure that didn't leak coolant, but i guess there's a very slim chance yours managed to be broken in such a way that the coolant doesn't fully circulate, i don't buy it.
