It's been my observation that clutch judder is usually most pronounced when starting off from a dead stop. This is because this is when you are "slipping" the clutch the most. When shifting gears, if you have the revs right, you don't slip it much.
Mmm, I wouldn't think the inner drivers (the green things) would be causing the problem at such low speed. I have worn inner drivers on my car and it shows up as a vibration on acceleration between 20 and 30 mph.
Another thought on what could be causeing the problem: engine mounts or the side strut mount maybe? These are what keeps the engine stable in the rotational direction.
