You can't really hear the difference but a good rule is that timing chains don't wear out but balance chains do (or actually the balance sprockets do).
As I have posted before. I remove the oil pressure reducer valve and check for scoring. I cut open the oil filter and look for metal debris. I can also remove the oil pump and inspect the pump gear, timing cover housing and oil pump cover. This takes about 1 1/2 to 2 hours to do.
If I see lots of scoring I know the balance sprockets are gone and it is no use of guessing and wondering what to do.
Off with the head, oil pan and timing cover.
If I don't find any scoring on the oil pressure valve or metal debris in the filter, I don't bother to remove oil pump since I know the engine is good and I let the customer drive it.
