All those things are real issues... the paranoid Saab owners are the ones who carry extra DI casettes in the trunk!
1) My "small" idler pulley fell of around 54k miles along with the belt. Since my model is earlier than yours and I have two of them (and did not know enough to replace both at the same time), same thing happened about a year later. Since it had to be fixed as an emergency repair, $200+ incl. towing each time (car overheats quickly with nothing driving the water pump). I am about to replace mine for the third time around 100k.
The pulley is about $35 online. You can reach and replace the upper one yourself. Sad part is that each time I heard an intermittent noise from the belt area and took the car in, only to be assured everything was ok. The second pulley took an almost new belt with it... (!)
2) I don't know about every 20k, but I never did anything to the rear brakes except new pads... until one rear caliper rusted solid at around 95k miles and 8 years.
3) Dean pretty much said it.
