Does the temp indicator show a normal reading?
Does the rad fan work properly?

Could be a problem from overheat caused by fans, controls or t-stat.
Have you checked all of of the hose clamps in that area and checked hoses for damage? If hoses are original, that might be part of the problem. Heater hoses seem to be the most prone to failure.

Check that the rubber seal in the reservoir cap is there and undamaged. Check for a crack in the reservoir.

The empty reservoir fills when you check it perhaps from steam in the (overheating) head. You need to fill when things have cooled down for a while. Drive a bit, and refill etc. The rad does not trap air to an extent that any special measures are required.

Has coolant been maintained/flushed as part of preventative maintenance? Rads used to become restricted from deposits when that coolant changes were neglected.

If you have ever had the small hoses removed to do a t-stat replacement, then these really have a problem resealing when clamped again with the oem gear clamps. The solution is to get the smaller clamps from the hardware store and put a pad of duct tape folded over on itself between the clamp and the hose to prevent the sharp edged slots in the clamp band from cutting and extruding the rubber through those slots. The OEM clamps simply have too large of a rigid section to develop properly distributed clamping forces when used again on an old hose distored from prior clamping.

The t-stat housing might be leaking where parts clamp on the t-stats seal ring. The pressures there are not very high. There can be corrosion and deposits in the seal area that cause problems after a new t-stat is fitted. Once the two metal parts are bolted up face to face, making the bolts tigher will not increase the pressure on the seal, but the bolts can be busted or stripped. A new seal may be needed, and the seating area cleaned up. IF the t-stat seal sits in a recess in the cap, the cap could be lapped to increase the clamping crush on the seal. But in any case, remove any deposits that will keep the two parts from fitting tightly.
