About every 2 years is the rule, but few people actually do it that often. And even if it's part of a major maintenance, I suspect many techs look at the fluid and if it's even semi-clean let it go. Bleeding is a nuisance for most techs, no matter how they do it.
I've seen that the stuff definitely gets dirty over time, many heating and cooling cycles, absorbed moisture, etc.
And if you do track days, most organizations require clean fluid and some want documentation that it was changed. I've done track days with a BMW club and they want to see fresh fluid within 90 days of the track day.
