I had an 89s that had been converted to 5spd by a PO without changing the carrier bracket. Because it changed the alignment, it vibrated terribly and ovalized the bearing seat for the stub shaft in the transmission. I put in the correct M carrier bracket, reset the bearing in JB weld, and all was good for 60k miles.
In your case my WAG is that it's either due to a lot of high torque hammering, assembly misalignment at some point, or dirt.
