Intermediate shaft splines worn away AND...(long)

the correct carrier bearing was installed. It is a 97 Aero 5 speed. I was taking off from a stop sign and heard a light pop before it just wouldn't go forward or reverse. The engine was running. The car rolled fine but all I would get was a light gear grinding sound when I let the clutch out. I thought something let go in the clutch so I pulled the tranny and found the transmission end of the intermediate shaft had hardly any teeth left in the spline. The clutch seems fine. I pulled the carrier bearing and support thinking I was going to find an "A" marked on it but found the "M". So I can't blame that. Not sure what to blame. I'm a little reluctant to just throw it back together with a new intermediate shaft without knowing what went wrong. I may just end up here again. Is it likely that I have a worn bearing in either the carrier or on the transmission output shaft? The output shaft doesn't seem to have much play radially. It does clunk in and out a bit. Can anyone tell me how much end play there should be? If anyone has any theories on this I'd sure like to hear them....
