bad without throwing a code: maybe means that it hasn't thrown a code yet. a specific sequence of events has to happen before a code lights up stays up. first the malfunction is in "temporary" type memory (for lack of better term). at this point the light is not lit. when the # of times this malfunction event happens equals the # of times to become a "permanent" type code the lamp will stay lit. read your service manual for specifics.
running rich can be many things. a real time scanner can help determine the cause. IAT sensor, thermostat, MAF, weak spark, bad valve, etc could also be culprits. if the cat is partially plugged, usually performance will suffer and many different codes will result. it may not even be an o2 sensor.
if it were me: physically clean carbon off of everything. check plugs. put your magic potions in. have not had experience with the potion you used. techron by chevron has something other than white gas in it that actually works. then change plugs.
