It's not only the tensioner pulley that fails.
I have seen both the center and lower idler pulleys fail even though it is not as common as the tensioner pulley.
In this particular car, the belt was not even due for another 12K miles.
Most car manufacturers have a time and miles limit on their timing belts.
For the Volvos (which have used timing belts since 1976) it used to be 50K miles or 5 years for the old 4 banger but have since 1999 moved to 10 years and 105K miles.
The belt in itself in the Saab V6 engines is really strong and would easily last 100K miles but for some or another reason the bearings in the pulleys just don't agree. That's where it went wrong.
No, it's not that simple for a car owner to do any precautionary inspections.
This particular car I saw at 44K for a service and it didn't have any warning signs whatsoever that the pulley was going to fail in another 4K miles.
Another car that I had in for an oil change in the end of a week had some minor noises and that next Saturday, the tensioner failed leaving the pulley looking like this.
