I used to be into the car stereo thing in college- my limiting factor was power consumption. I went through 3 alternators in 2 years trying to perfect my setup. Higher output alternators were expensive and hard to fit within the engine bay on my car ('88 Accord)
After 3 years of trying (and saving) I finally got the system where I was actually satisfied with it. I had it that way for about 6 months and someone decided that they wanted it more than me- and took it. Since then only factory stereos for me. I've had the Monsoon system in a Jetta, and now the HK in the Saab. Both decent systems, but nothing compared to what can be achieved with aftermarket equipment and a ton of cash and effort.
Ambient noise is a big concern when dealing with auto sound systems. One way to overcome this is to turn it up! (or, I guess you could use more sound insulation and quieter muffler) I was one of THOSE guys with the Honda- lowered, big wheels, and the obligatory grapefruit shooter exhaust to compete with the (loud) stereo. The noise was a bit intrusive around town, but cruising on the highway with the radio turned up sounded great.
One funny thing I noticed- with the combination of the loud exhaust and certain bass frequencies there would be a phase shift and the stereo and exhaust would cancel each other out! That was a very interesting side effect of combining ambient noise levels with those from the stereo.
