Lose an entire expansion tank of coolant from bad cap?

About 5 months ago i did a coolant system flush but the car only took about 1 gallon of coolant/water mixture (didn't drain from engine which is what i attributed the "only 1 gallon" to... system capicity is about 2 gallons).

This weekend i opened the hood and the expansion tank was empty. Car has been cooling fine and not running hot at all, but was surprised to see the tank empty. I refilled and after driving around a bit i looked under the hood and saw a little steam coming from the cap area, but not certain if the cap is faulty or this was just some spilled coolant burning off from when filling up the expansion tank.

My first fear was a head gasket (we read so much about it on the board) but i opened the expansion tank after sitting overnight and there was no pressure.... so that was a RELIEF.

Just trying now to figure out where the coolant went, how much could be "lost" from a faulty expansion tank cap, OR perhaps the coolant went to fill the heater core? (although i have the dial all the way to HOT when i did the system flush, i did not have the blower dial selected "ON").
