I also have a '91 9kt, which I bought with 110K miles on it. The tranny case had been cracked by hitting some object, so the first thing I did was get a rebuilt unit from Eriksson. The dealer put it in and insisted on using the new Saab synthetic transmission lubricant. After about 2K miles, I started getting the same symptoms, only if I took it over 4K rpm's in 1st gear prior to shift. Even if I shifted gingerly and slowly, it still happened. I could only prevent it by double clutching.
Eriksson replaced my tranny and the dealer again installed it, again with synthetic lubricant. I drove the car more gingerly, and after 7K miles I got the same problem again.
It is due to wear of the 2nd gear synchro. It's not the shift coupler, not the dogbone, not your shifting technique. You are showing the first signs of wear, and you can coax some more life out of it by shifting at lower rpm.
If you have Saab manual transmission lubricant in it, I would recommend you get rid of it. Eriksson now has a policy of recommending against this lubricant for earlier 9000 trannies. Mine was not the only tranny to suffer this fate, but it was one of the first that they experienced, since my tranny was put in immediately after the new lubricant came out. Of course, the mechanics were hot to use it.
If you need to change lubricant, be advised that a complete replacement is necessary, and this isn't easy in '91. I can provide you with more details if you need them.
