Hate to invoke a more expensive fix, but since you mention that:
a. Brakes pads aren't making full contact with the existing rotors, and
b. One rotor seems more worn than the others
I would take a look and see how your caliper is behaving. You may get a good idea if it's seized or binding if it's unduly hard to contract when you're installing the new pads.
Even with a fairly worn rotor, the brake pads should wear themselves down to match and continue to make full contact. If that's not happening I suspect a stuck caliper.
Of course, this was the one rotor you didn't replace before, so that may explain the ridges, but the contact issue is another matter.
