Sounds like cam chain to me

If by 'grinding' you mean a metallic rasping sound that varies with engine speed... well, my one excursion into 99 ownership (a '74 99LE, IIRC) made the same kind of noise and it was a loose timing chain, aka cam chain. If the chain is loose enough to throw off the timing, that would make it run weird, too, as DougM says.
The mechanic who ID'd this for me told me the engine would have to come out to fix it properly, but also suggested it might be more interesting to leave it alone: "I've heard that if it lets go at highway speed, it'll whip around and slice the engine in half just like a chain saw... I've always kind of wanted to see that." I solved the problem by an alternate method -- I swapped the 99 for a Ford Fiesta and swore that I'd stick to 2-stroke or V4-powered Saabs thereafter!
