I forgot one important point. The additional variable is wear on the shift fork itself. As the contact face of the fork wears, ther's a reduction in engagement depth of the sleeve onto the gearwheel. If there is significant wear, which can be the result of resting your hand on the shifter when in gear (only if exerting, say, slight rearward force in second for example), then little mainshaft thrusting is needed to disengage. The total depth of engegement is only about 0.100 inch. I routinely see 1/2 forks worn on the second gear side in the range of 0.030 - 0.050 in.
Another message in this: holding it firmly in second to prevent disengagement is actually exacerbating the problem through accelerated fork wear. All the while, at the far end of the shaft, 4th gear is grinding and welding itself into ruin. Sufficient play in the pinion bearings also ruins the ring and pinion gears once perpendicular alignment can no longer be maintained.
Moral of the story: fixing it sooner rather than later will save you a lot of money.
