I'm not sure (without going and measuring and such) how far a slave has to travel in order to "blow out" when installed but here's a thought. First, as noted, if the slave is used to install the spacer tool then it should work fine for removing the same tool from the same clutch cover later (duh). If a guy was replacing his clutch cover with a new one and installed an overly thick improvised spacer tool in that new cover using a press (or using the "jack up your car and use it as a press" method) having that overly thick spacer tool in there might allow excessive travel of the slave piston. Maybe that explains the phenomenon. Either that or someone just forgot to install their throw out bearing (picture that mess). I like turtles.
