The 9000 engines have two coolant temperature sensors (CTS):
1. sensor for the gauge is located on the transmission end of the cylinder head
2. sensor for the Engine Control Unit (ECU) is on the head located between the intake runners for cylinders 2 and 3.
Starting in 1995, the fan is controlled via the CTS that gives information to the ECU, which processes it and controls the fan operation via two relays located in a box in the engine compartment. Note that the fan has two speeds; to run at the slower (starting?) speed the circuit goes through a large resistor mounted on the radiator fan support. There is a lot that can go bad here, have a look at the link for troubleshooting info - but know that your car is newer and has the change in fan control from the sensor on the radiator to the ECU.
