Do you have a working free wheel or is it locked out? The free wheel will usually slip under acceleration in 4th gear. If you have to manually move the shift lever back into 4th after it pops out to re engage the transmission you probably have a problem inside the tranny with the shaft nuts that have gotten loose over time to allow 4th gear to move away from the syncro.
