I do have a working free wheel. It has worked without trouble for about 10,000 miles.
I make sure it is locked in or out. But this is not "freewheel" this is "neutral"when it pops out.
Maybe a detent spring broken or like you say, shaft nuts loose on the transmission.
I drove it to work and back and no trouble but I shifted slow and smooth and no on and off the gas hard.
This is my daily driver , I have rebuilt from the ground up, so I take it easy most days on it.Don't want to destroy all my hard work.
