Per GM's letter to NHTSA, Quality Package 3 (the TK 3.6 4 cylinder DI) went into production in July, 2002. Units made before that which were in the spare parts distribution system continued to be sold until they were all gone, while '2003 4 cyl 9-5s (and 9-3 CVs) were treated to the latest design. Quality Package 4 was the software upgrade, which was available in autumn of '02 IIRC. Saab's service policy specified that the software update be installed (and the new diagnostic run) prior to the warranty replacement of a DI cassette. Techs at the time grumbled about it - as the nose knows when a burned DI is pushed through the door. As it turns out, most of those software updates were simply advances on the recall.

It's kind of shitty that it took 3 dozen owners complsining to NHTSA to kick start the recall process. The speed and level of detail with which GM answered NHTSA, and the two-order of magnitude reduction in failure rates provided by the latest systems suggest that a voluntary recall should have been initiated by GM by now. On the brighter side, they certainly seem to have nailed down the cause and cure for the problems.
