Your boost seems a bit low to me, given the mods listed. Though, that's not to say it's bad, if everything is smooth (unlike my car which is smooth when it wants to be but goes like stink when it has full boost). :)
Remember, stock max boost for your car should be 11 psi, so 12 psi isn't that much more power for all you have listed. You are correct though, in that 92 octane should definitely help sustain higher boost levels. On the other hand, if you are sustaining those 10-12 psi boost levels to redline, that's still significantly better than the stock APC, which may hit 11 psi on a good day, but will immediately start tapering as RPMs rise, down to as little as 6 psi by redline.
No idea why you have no boost in 1st and so little in 2nd. Unless I'm wrong, and I'm never wrong (sorry, couldn't resist!), there is no circuitry in your car which would limit boost per the current gear selection. I get max boost in both 1st and 2nd, though that is quite unusable with snow tires on, and not so usable with high performance summer V rated tires, either. Now slicks, maybe...
You might check or have someone check your base boost, a low base boost can certainly account for low max boost. In my car, base was only about 4.5 psi or slightly less, upping it just 1.5 psi raised max boost about 5 psi. I may be turning it back down for solidity sake though, if I can tame it via other means.
