I don't see how the cam can have moved (I didn't remove it).
I set the engine to tdc, bolted the sprocket to its bracket and removed the head. When I went to re attach the head the engine apeared to be about 5deg, so I moved it back to TDC, asuming I had moved it in the removal / cleaning process. The spocket wheel wants to sit slightly to the right of the cam shaft (bolted to it's bracket). When I move it across to the cam it seems to be slightly out of allignmnet. - I can see the slight rust marks from the gear's old possition and there not quite the same. By the look of things if I rotate the engine slightly it will align and then bolt on (with effort). But then the timing won't be correct. I really don't understand what has happend here, my theory is that the chain may have sliped a notch. Not sure how, the tensioner is tight and while the head removal was not clean it wasn't all that brutal either.
