Personally, I would DIY and save a lot of money. Out of your list, here are the items I've done, and how much the parts cost:
Heater core $70 off eBay (since you're going to be in there, spend another ~$60 for a blower motor and do it at the same time)
Radiator $50 used (I have owned quite a few 9000s and the only reason I ever had to replace the radiator in one was because I wrecked it... so I am comfortable with my odds buying one used)
Upper rad hose ~$20
Drive belt ~$30 (but you should also replace tensioner and pulleys if they haven't been done for along time. you can use a 6" C-clamp to compress tensioner, you don't really need a special tool)
Power steering and brake flush ~$20
If you have never replaced the fuel pump, I recommend replacing that preventatively as well, ~$100.
